Liability Waiver

On behalf of myself and/or the minor set forth below, and my personal representatives, heirs, next of kin, executors, representatives, assigns and administrators (collectively, “I,” “me” or “my”), I ACKNOWLEDGE THAT MY PARTICIPATION IN LESSONS, EVENTS, AND OTHER ACTIVITIES, USE OF EQUIPMENT, AND ACCESS TO FACILITIES (“ACTIVITIES”) AT GIFTS OF MUSIC NW LLC IS VOLUNTARY AND MAY INVOLVE RISK OF BODILY INJURY, DEATH OR DAMAGE TO PROPERTY. I ASSUME ALL SUCH RISKS, BOTH KNOWN AND UNKNOWN. In consideration of Gifts of Music NW allowing me to participate in Activities, I release and hold harmless Gifts of Music NW and its directors, officers, teachers, managers, and employees from all rights, claims, demands, losses, damages, expenses, costs and actions (including reasonable attorneys’ fees) that I may have in connection with participation in the Activities, including bodily injuries, exposure to diseases, viruses, bacteria or illnesses or the causes thereof, illness, death, personal injuries or property damage that I may incur or that may relate to my participation in the Activities, whether or not they result from the negligence of Gifts of Music NW Parties or others. My participation in Activities is subject to the rules that Gifts of Music NW may require, and I will be obliged to pay for any damage that I may cause while participating in the Activities.

I have read, or have had read to me Gifts of Music NW lesson policy, and understand this document, and I am familiar with its contents. No oral representations, statements or inducements have been made to me by any Gifts of Music NW Parties. This document will be construed in accordance with and governed by the laws of Washington and is intended to be as broad and inclusive as possible. If any portion is held invalid, the remainder will continue in full legal force and effect.
